چکیده

The abnormalities of pulmonary arterial blood supply in mitral stenosis, consisting primarily of constriction of lower lobe vessels with resultant upper lobe preferential flow, was assessed by means of pulmonary angiography in 30 patients with mitral stenosis uncomplicated by other cardiac diseases. The abnormalities in blood flow correlated best with both the pulmonay artery pressure and the pulmonary arteriolar resistance, and only reasonably well with the area of the mitral valve and the pulmonary capillary wedge pressure. It is suggested that the reversal of the normal lower lung field preferential blood flow in mitral stenosis is primarily a function of pulmonary artery hypertension due to increased pulmonary arteriolar resistance. Similarly, the width of the right main pulmonary artery as measured on pulmonary angiograms correlated well with both the pulmonary artery pressure and pulmonary arteriolar resistance and less well with the area of the mitral valve. Pulmonary artery size in mitral stenosis is most likely a function of the severity of the pulmonary arteriolar resistance as reflected in the pulmonary artery pressure.

بررسی عوامل موثر در افزایش سطح دریچه و پیدایش نارسایی میترال بعد از بالون میترال والولوپلاستی در 110 بیمار در مجتمع بیمارستانی امام خمینی

Background: Percutaneous Transeptal Mitral Commisurotomy (PTMC) has been used in treating mitral stenosis with variable success, its main complication being mitral incompetence. There is a need to define the subgroup of the patients who benefit mostly from the procedure. Methods: We studied 110 patients (age 17 to 60 years mean 33.2) with mitral stenosis. PTMC was performed though femoral vein....

توانایی اکوکاردیوگرافی داپلر در تخمین فشار پرشدگی بطن چپ در تنگی دریچه میترال

Background: Estimation of Left Ventricular End Diastolic Pressure (LVEDP) among patients with Mitral Stenosis (MS) helps to diagnose diastolic dysfunction and to explain their symptoms. However, Conventional Doppler measurements have limitation in predicting of LVEDP. This study sought to establish whether the correlation between measurements derived from Tissue Doppler echocardiography and LVE...

The natural history of mitral stenosis.

The relationship between hemodynamic abnormalities and disability was studied with paired cardiac catheterizations nine to 69 months apart in ten patients with mitral stenosis. Left atrial and pulmonary arterial pressures and pulmonary vascular resistance increased in each patient; however, there was no consistent relationship between the progression of symptoms and the palmonary vascular resis...

Hemodynamic and clinical significances of atrial fibrillation, pulmonary vascular resistance and left ventricular function in rheumatic mitral stenosis.

Seventy-four patients with rheumatic mitral stenosis were catheterized and hemodynamic and clinical significances of atrial fibrillation, pulmonary vascular resistance and left ventricular function were studied. These data were also compared to those in the 6 control cases. In addition to the correlation of mitral valve area to the functional classification of patients, significance of atrial ...
